SB12 Annual Consent Form
During the 89th Texas Legislative Session, lawmakers passed Senate Bill 12, which requires schools to get annual parental consent before providing routine physical or mental health services. This includes things like first aid, short-term counseling, or social emotional support. To complete the consent form in Skyward:
- Log into your Skyward Family Access account
- Locate the 2025-26 Student Handbook Form
- Indicate whether or not you consent.

Dear Sheldon Lake Family,
Hello! My name is Mrs. Gutierrez and I am very pleased to serve as the counselor at Sheldon Lake Elementary. I am a graduate from the University of Houston and completed my graduate program in counseling from Lamar University. I look forward to working with your child!
My goal for this school year is to help each of our students have a successful and positive year. I will be providing whole class guidance lessons as well as individual and small group counseling services to all students.
I look forward to getting to know all of our students and their families. Please feel free to call me if you have any questions or concerns about the counseling program or your child. By working together, we will succeed academically, socially and emotionally.
